Your First Visit: $99
Your first visit starts with a conversation about what brought you in, followed by an assessment to help us understand what’s going on and make sure the care you booked is appropriate for you.
If you’re here for chiropractic care, your visit may include an adjustment. If you’re here for shockwave therapy, treatment may begin at your first visit.

Please complete your new patient forms before your appointment. We promise we read them! Dr. Leslie reviews your forms ahead of time so she can come into your visit prepared.

Once you arrive, we’ll guide you through the rest. We leave plenty of time in your first visit for questions, so you never have to feel rushed.

How to Prepare for Your Visit
Our office is very much a come-as-you-are kind of place. Wear whatever you’re comfortable in and whatever makes sense for the rest of your day.
For shockwave therapy, we’ll need access to the area being treated. If what you’re wearing doesn’t work for that, no worries! We have clothing available for you to change into.
Please complete your new patient paperwork before your appointment so Dr. Leslie has time to review it ahead of your visit.
If you have recent imaging or relevant medical records, please bring them with you or send them ahead of time.
Destination Chiropractic is located at 2935 Baseline Road, Suite 300 in Boulder.
We have free on-site parking, and our office is accessible by both elevator and stairs.
Once you enter on the main level, head up one floor and you’ll find us down the hall.

Before you leave, we’ll let you know what the next step looks like.
If you’re here for chiropractic care, that may mean scheduling a follow-up Report of Findings visit, arranging x-rays if needed, or referring you out if something requires a different kind of care. At your follow-up, Dr. Leslie will review what she found and go over your recommended care plan with you.
If you’re here for shockwave therapy, we’ll let you know how many treatments we recommend based on your first visit so you can schedule accordingly.

Destination Chiropractic is an out-of-network practice. We keep our pricing transparent so you know what your care costs before moving forward. For many private insurance plans, we can provide a superbill that you may submit for possible reimbursement.
Were you injured in a car accident? We may be able to bill MedPay for eligible auto accident care. Please contact us before scheduling so we can help you determine the next step. -
No referral is needed to book chiropractic care or shockwave therapy at Destination Chiropractic. You can schedule directly with us online, by phone, or by text.

That’s totally okay! If you’re unsure which service makes the most sense for what you’re experiencing, reach out before booking and we can help point you in the right direction.

We’ll tell you. Your first visit is also about determining whether the care you booked is appropriate for what’s going on. If Dr. Leslie thinks you need imaging, another provider, or a different type of care, we’ll help guide you toward the appropriate next step.

Not everyone needs x-rays before receiving chiropractic care. Dr. Leslie will determine whether imaging would be helpful based on your history and exam. If x-rays are recommended, we can help you with the next steps and coordinate with a local imaging center.

If it is appropriate based on your history and assessment, yes. Chiropractic first visits may include an adjustment, and shockwave therapy can typically begin during your first appointment. If Dr. Leslie feels that additional information, imaging, or another type of care is needed first, we’ll explain that clearly.

Absolutely. You’re welcome to bring a partner, friend, family member, or support person with you if that helps you feel more comfortable during your visit.

The number of shockwave treatments varies depending on the area being treated and what is going on. After your first shockwave visit, Dr. Leslie will give you a recommended number of treatments so you can decide how you’d like to schedule your care.
